Clinical Case Manager (RN)
(In-home Nurse with Field Responsibilities)
 
This hybrid opportunity involves managing a caseload of medically fragile patients while ensuring that the clinical care delivered by our field staff meets or exceeds the Aveanna Clinical Standard of Excellence. Our ideal candidate will be compassionate, demonstrate excellent bedside manner, and have a strong interest in pediatrics and private duty home health.
 
Key Responsibilities:
  • Field-Based Work (75%): Spend quality one-on-one time with patients, providing direct care and addressing their medical needs in the comfort of their homes. You will play a crucial role in developing, mentoring, and offering professional guidance to our field staff, ensuring that the highest standards of care are maintained.
  • Office-Based Work (25%): Conduct clinical supervisor duties, including internal case management, assisting with hospital discharges, managing the start of care process, validating nurse competency, and completing supervisory visits.

Schedule: Monday – Friday, 8-hour day shift. You will not be on-call initially, but will work up to it. Once on-call, it will rotate every third week, possibly every other week.
Work Location: 3421 N. Causeway Blvd Suite 502 Metairie, LA 70002, Corresponding Patient Homes, and Office Work as Needed.
Compensation: $70,000.00 base salary plus monthly bonus potential, mileage reimbursement, and a cell phone stipend.

Responsibilities of the Clinical Supervisor:
  • Internal case management, evaluation, and development of nursing talent
  • Participating in patient releases from facilities to their homes through “Start of Care Assessments” and “Re-evaluations”
  • Reviewing physicians’ orders and updating plans of care and documentation as necessary
  • Assisting with interviewing, selecting, training, and validating new clinical staff
  • Promoting and enhancing the clinical expertise of staff to ensure safe and quality nursing care for our patients
  • Collaborating with the operations and business development team
  • Ensuring adherence to State, Federal, Local, and Aveanna policies and procedures
Qualifications of the Clinical Supervisor:
  • Must have a good-standing RN license in the state where you will practice
  • Must have at least 1 year of hands-on nursing experience
  • Must be able to travel to patients’ homes in designated territory
  • Must have reliable transportation, a valid driver’s license, and pass an MVR check
  • Current CPR certification from AHA or ARC
  • Must be willing to supervise nurses providing in-home skilled nursing care to infants, children, adolescents, and/or adults
