Job Summary:
Coordinates the Quality initiatives related to the Mary Washington Healthcare Thoracic program at both State and National levels. Abstracts and organizes data obtained from concurrent and retrospective chart review of thoracic surgery patient subsets. Inputs data into the national Society of Thoracic Surgery (STS) database. Performs outcomes analysis, identifies trends, initiates review and assists with coordination of MWHC quality improvement team meetings. Generates outcome reports with dissemination to Mary Washington Healthcare committees, Thoracic Surgeons, and managers. Ensures data integrity and timely harvest according to Society of Thoracic Surgeons (STS) definitions/specifications Collaborates with Manager to ensure validity of data. Acts as educational resource for other members of the Quality Improvement team. Performs chart review for current PI projects. Represents the organization at conferences, collects and validates data for state reports.
Essential Functions & Responsibilities:
- Responsible for capturing, validating, and inputting data into the STS database for both the Thoracic program.
- Assists with abstractions and entries into the TVT and thoracic surgery database.
- Interprets/analyzes data to determine trends, identify causative factors, and determine opportunities for improvement.
- Generates outcome reports with dissemination to appropriate committees, managers, service line directors and MWHC Leadership.
- Ensures that all functions of the registry are met for each accredited program using the guidelines set forth by the Society of Thoracic Surgeons.
- Provides/reinforces disease-specific education to patients, families and caregivers to support patient self-management and achieve optimal clinical outcomes.
- Provides pre-operative patient education, as applicable.
- Provides assistance with thoracic clinic, as applicable.
- Analyzes and identifies readmission trends through chart review, patient/family interview and staff engagement, and develops processes to reduce readmissions.
- Communication of relevant patient and plan of care information across care continuum
- Maintains and coordinates updates of STS Database; communicates with relevant STS parties.
- Coordinates data cleanup and data harvests on a scheduled basis.
- Validates registry databases and data prior to submission to appropriate registries.
- Schedules quarterly sessions, with surgery physician leaders to update them on the progress of the STS data input and discuss data reports.
- Provides thoracic data for Quality Department as needed.
- Responsible for maintaining own current competency file.
- Demonstrates competency in emerging STS treatment and diagnostic modalities.
- Participates in Mary Washington Healthcare STS policy and protocol development.
- Performs other duties as assigned.
Qualifications:
- Graduate of an accredited nursing program required, Bachelor’s degree in Nursing (BSN) preferred.
- Valid RN License from Virginia or reciprocal compact state required.
- Minimum of two years clinical experience required.
- Minimum of two years thoracic experience (thoracic surgery, thoracic care) preferred.
- Performance Improvement and Quality Management experience preferred.
- Must have thorough knowledge of cardiovascular anatomy and operative reports.
- Must have advanced computer skills to include data input, Excel, Outlook, Microsoft office, electronic medical record, spreadsheet skills, patient care information systems, data management and analysis, and other required software.
- Must be skilled in medical record review using established abstraction guidelines.
- Must possess clear communication skills (written, verbal, and presentation).
- Must demonstrate understanding of the approaches and methods of performance improvement.
